Supplemental Evaluations

A supplemental evaluation is granted by the Associate Dean of your program area. If you qualify and receive permission from your Associate Dean to complete a supplemental evaluation in the Test Centre, please complete one of the appropriate online forms below to submit your request.

Supplemental Evaluation Forms

Students attending the Welland Campus are to complete and submit the Supplemental Evaluation Form - Welland Campus.

Students attending the Daniel J. Patterson Campus in Niagara-on-the-Lake are to complete and submit the Supplemental Evaluation Form - Daniel J. Patterson Campus.

Things to Remember

• Niagara College students who do not attend or cancel within 24 hours of their scheduled testing appointment will have a fee of $20 applied to their student account.
• Cancellations are to be submitted via email to your campus's Test Centre. Cancellations over telephone are not accepted.
